Achieving predictable esthetic outcomes for direct composite restorations with injection overmolding

Lecture Description: 

Direct restorative options for smile repair or enhancements have the advantage of conservation of tooth structure. Unlike indirect restorations, there is no need for path of draw and insertion, and uniform material thickness. Thus, tooth structure can be maximally preserved. Challenges with layered composite restorations can be shorter life span, difficult skill acquisition and mastery, and a dependence on clinician artistic and artisan skill. The trend with indirect restorations is leaning towards monolithic milled ceramics. Injection overmolding is a novel method for encasing a tooth within monolithic composite resin. The result is essentially a monolithic hand-milled composite resin restoration created directly within the mouth.  The advantages are greater strength as compared to layered composite resin restorations, easier skill acquisition, simpler shade selection protocol, and minimal inventory needs for a wide range of composite shades and opacities. Monolithic injection overmolded restorations require simple but different shade selection considerations as compared to layered restorations. Understanding the impact of air gaps within the restorations, surface texture and polish, and composite thickness are important factors of the predictable shade selection protocol for injection overmolded restorations.
